QCHealth™ is Bridging the Known Engagement Gap Between Patients and Pharma-created Services

QCHealth—a division of Aventria Health Group, LLC—is a cause-driven organization. Founded earlier this year, QCHealth is dedicated to improving patient outcomes by doing what we can to solve an indefensible problem: patients do not follow an optimal care plan. There are a multitude of reasons for this, but the ones that QCHealth addresses are:

  • Lack of time. Provide time to investigate and discuss the right path and equip the patient to engage in shared decision-making.
  • Standardized care is a must for consistently better outcomes, so unwarranted variability is a clear and present problem. It’s impossible to expect everyone in any office practice to be aware of, and implement, the most current clinical practice guidelines. Further, patients may not have the information needed to report worrisome side effects, schedule necessary follow-up care, or practice healthy behaviors.
  • Searching patient education libraries is arduous. Providers should have the right tool at the right time for ideal patient care planning and engagement. It is preventable inefficiency if each individual working with patients is taking the time to curate their own mix of educational tools for patients.

QCHealth offers Outcome Guided Engagements (OGEs) that provide the right tools at the right moment for enhanced patient care. This information is offered FREE to nurses, doctors, case managers, and others on the frontlines of care working hard every day to better equip patients to become a co-author of their own care journey.
