Case example
Let’s consider an example that highlights the power of this approach. People with diabetes are at high risk for chronic kidney disease (CKD), yet shockingly, 2 out of 5 individuals with severe kidney disease are unaware of their condition. These patients often “crash” into dialysis in the emergency room, receiving treatment that their bodies haven’t been properly prepared for. Clearly, this is not an optimal approach to patient care.
So, how do we fix such a monumental problem? The key lies in isolating the care gaps that contribute to this situation and finding a small yet impactful problem that we can solve.
QC-Health®, a cause-driven benefit corporation affiliated with Aventria Health Group, took on the challenge of CKD patients crashing into dialysis. We assembled a team of passionate stakeholders who manage diabetes patients, all driven by the intrinsic motivation to better the lives of their patients. Through extensive discussions surrounding countless care gaps, we identified and focused on one specific problem.
We are addressing the fact that even though patients with diabetes are at increased risk for developing CKD, they often don’t have the proper screening tests done during their annual exam. Without the proper tests, they can’t be staged and treated appropriately to prevent disease progression.
With our solution now in the final stages of development, we are excited to launch it in a pilot phase this fall. While we haven’t completely solved the issue of CKD patients crashing into dialysis, we have made incremental progress moving the needle in the right direction. We’ve engaged passionate stakeholders, sparked a movement, and laid the foundation to replicate our success across different regions of the country.
