Improve CKD Care by Fostering a Groundswell With MA Plans

There are a host of unmet needs and care gaps in chronic diseases that the Centers for Medicare & Medicaid Services (CMS) is trying to address. Medicare Advantage (MA) plans, in particular, are working to improve the cost of care and patient care and are looking for opportunities to collaborate.

Medicare Advantage plans have higher enrollments among minorities, specifically African Americans and Hispanics, compared to Caucasian Americans. This presents a valuable opportunity to target care gaps and improve outcomes in these populations with poor health results, making it an ideal area for collaboration with MA plans.

Groundswell Movement in CKD: An Opportunity for Improvement

We identified that a significant care gap exists among patients with diabetes and chronic kidney disease (CKD), particularly among minority populations. This vulnerable group encounters high treatment costs, poor outcomes, and a strong association with heart disease and other coexisting conditions.

Our CKD groundswell movement aims to meaningfully improve patient outcomes and change the course of disease progression. We are on track to do this through earlier detection and intervention by primary care, which ensures the overwhelming number of patients with diabetes who are unaware they have CKD are diagnosed. Doing so allows for appropriate interventions before CKD progresses to the point of being symptomatic, when it is much more severe.

What we are calling a groundswell movement is a win-win-win, where all those involved are focused on improving patient outcomes and achieving their organizational goals. The CKD groundswell movement is an example of something we started grass roots as a provider-identified problem where we created a solution to address the problem.  Collaboration creates a way to directly partner with MA plans, other payers, and a variety of healthcare stakeholders.
